Puppy Adoption Application Form

Thank you for your interest in a Sierra Vista Labradoodle or Goldendoodle. The purpose of this application is to ensure the best Owner/puppy match possible. To achieve this we ask you to be as complete as you can with all responses.

To reserve a puppy, this application form must be completed on-line. Once the application is reviewed and accepted, you will be notified and then a deposit (non-refundable) of $250.00 will be required to secure your space on the waiting list.

Deposits can be made through PayPal (please see the PayPal link on our website) or you may send a check or money order to:

This deposit will be applied to the total purchase price of the puppy. An application received without a deposit will not reserve a puppy. We reserve the right to decline any application. If your application is declined you will receive a full refund of your deposit.

1. Are you interested in a Labradoodle or Goldendoodle? Why?


2. Is your doodle to be a pet or are you an established breeder applying for a breeding dog?


3. What size doodle would you prefer and why? Standard (45lbs plus), Medium (30-45lbs), Mini (29lbs and less)


4. Would you prefer a Male, a Female or Either. Why?

6. Do you prefer a doodle with a wool coat (more poodle like), fleece coat (looser curls) or straight hair coat (more retriever like)?


7. Does anyone in your family have allergies to dogs?

9. Will this be the first dog for your family?


10. If not, what dogs do or have you owned?


11. Do you own or rent your house, apartment, condo, other?


12. Do you have a fenced yard? If not, how do you plan to confine/protect your dog?


13. Are you familiar with crate training and are you willing to use it to train your puppy?


14. Where will the puppy be kept during the day?


15. If you work, will the puppy be alone all day? (Yes/No)


16. If yes, will he/she be taken out for a mid-day break and play time? (Yes/No)


17. Owning a dog requires an investment of time, energy and money. Are you willing and able to accept that responsibility? (Yes/No)


18. What is the name of the veterinarian or clinic you will be using for the puppy?


19. Have you used this vet before? (Yes/No)


20. May we contact them for a reference? (Yes/No)


21. We request that a dog be returned to us, at any point in time, should you find yourself unable to care for it. Are you willing to agree to this? (Yes/No)
